The complete sacrament record is compiled at the child’s parish of Baptism. Therefore, part of the registration includes providing us with a copy of the child's baptismal certificate. If the church of baptism is not St. Ambrose, a copy of the candidate’s baptismal certificate must be provided as part of this application. You can email a copy of the baptismal certificate or stop by and the office can copy it for you.

- Safe Environment Background Check Cost - This cost helps to defer the cost of criminal background checks that all volunteers of the program are required to have. $5 per child. Each background check costs the parish approximately $118. REQUIRED

- First Communion-related fee - This cost covers the materials for First Communion banners (down to $5 with the assistance of a volunteer) and a photographer for the First Communion Mass (who makes all of photos from the day available to everyone). REQUIRED Please fill out this field.
